Step 2: Download the request form, complete it with all the requirements.
Don’t have access to a computer or an email address? Call our COVID-19 Hotline at 1784- 534- 4325 and our operator will be happy to submit the form on your behalf.
Step 3: Submit a copy of your travel itinerary and a copy of the Bio page of your travel document ( ID or passport ) and your request form to [email protected] at least 5 days prior to your travel. This time would allow us to offer you guidance and book your appointment.
Step 4: Be Patient. Once you’ve submitted your application along all the supporting documents, our officers will respond within 24/48 hours.
The cost of the PCR test is USD $20.00 OR EC $54.00 FOR CARICOM PASSPORT HOLDERS AND USD $40.00 OR EC $108.00 FOR ALL OTHER PASSPORT HOLDERS.
